- Cognitive-behaviour therapy (CBT) has established itself as the evidence-based psychological treatment of choice for a range of mental health conditions, and is is recommended in national treatment and policy guidances. However, the research trials which form the underpinnings of this evidence base have often needed to exclude participants with comobidities, particularly drug or alcohol misuse, for the purpose of simplicity and purity. Thus many CBT recommended treatment protocols and manualised approaches for common and more severe mental health problems have not been developed with those who experience comorbid mental health and substance misuse problems in mind. However, prevalence studies across a range of countries consistently indicate that alcohol and drug misuse are very common in those with mental health problems. Co-existing mental health and substance use are associated with poor engagement in mental health treatment and poorer treatment outcomes. An epidemiologic study carried out in the USA found that 16.7% of adults will experience alcohol or drug problems in their lifetimes. In contrast, for those who experience anxiety and depression the prevalence rates are significantly higher; 32% will use substances problematically and the figure is 47% for those who experience schizophrenia. For young people rates are higher still.
